We are seeking compassionate and motivated Healthcare Assistants to join a friendly Community Nursing Team in Gravesham. This is a fantastic opportunity to provide high-quality, person-centred care to patients in their own homes while working with a supportive and professional team.
This role is ideal for candidates with healthcare or support experience who are dedicated to improving patient wellbeing and contributing to a positive community healthcare environment.
Deliver high-quality, person-centred care to patients in their homes
Assist registered clinicians with clinical tasks, including observations, wound care, catheter care, and general health monitoring
Support patients with daily living activities to maintain independence
Communicate effectively with patients, families, carers, and colleagues
Maintain accurate patient records using electronic systems
Work independently and collaboratively as part of a multidisciplinary team
Provide care with dignity, respect, and professionalism at all times
Previous experience as a Healthcare Assistant or support worker in a clinical, care, or community setting
Strong communication and interpersonal skills
Caring, patient-focused attitude
Confidence using IT systems, including Microsoft Office (Outlook, Word, Excel)
Ability to work flexibly across a 7-day service schedule
Full UK driving licence and access to a vehicle for work purposes
